If you're looking for ideas, browsing on Pinterest may be helpful, as there are lots of lovely costume pictures on there.
For us to help you, some more information might make things easier - male or female? What age range? How much dancing do you need to be able to do in the costume?

If you are NOT looking for a professional ballet costume (assuming you are a girl):

White leotard
Long white wrap around skirt(above knee length)
Thick Black ribbon tied around waist
Black shawl
White and black mask

If you are looking for a professional ballet costume:

White leotard (unless connected to skirt)
Long white romantic tulle skirt
Black ribbon around waist
white or black gloves(optional)

If you are looking for a male costume:

Black tights
White long sleeve shirt (loose and with ruffles)
Black cape
White mask (half of a mask)

I'd go along with the above poster's idea of having a black/white leotard with a black/white romantic skirt.

Maybe have a masquerade-type mask and matching gloves, or if you wanted a prop in the dance, a masquerade mask attached on a stick. I always associate roses with Phantom of the Opera/masquerade, maybe some roses on the skirt or on the band of the skirt?

If you're going for a more Phantom look for guys, I'd probably do something with a mask and a cape.

How about a white skirt with a drape of black lace half covering it?

These could be nice: www.dansco.com . . .

Another alternative would be to get a basic dance dress in black or white, and decorate it with appliqued flowers:
